Identifying real IP

Is it possible to locate the real IP address and thus location if someone uses a proxy or TOR?

So there is no list to identify certain ranges and ISP's they belong to? Like for example international dialing codes are fixed and a list can be obtained

What's in an IP

New to the Forum and looking forward to learning more.

Does an IP nr stand for or mean something specific?

So if you look at an IP: 41.127.521.123 (Made up IP)

Does the 41 indicate something like an ISP and the 127 and the 521?
